I need to understand your situation a little better.

How have you determined where the messages are coming from?
How have you determined the messages are going through your server?

Well I Use GFI Mail Essentials So it shows that emails are being processed and it shows email coming from heaps of 'random' domains and getting sent to lots of 'random' domains

I'm not a GFI Mail user, so I don't know what it's reporting.

If the messages are going through your Exchange server, then Message Tracking should give you a place to start. See if you can determine from where the messages are coming, or to where they are going, and how far they get (are they being delivered?).

Check your SMTP logs to see if the messages are originating from outside your company. If so, you should check to see if you have an open relay.
