I keep seeing (unauthorized) users log onto and stay connected to the virtual server. Relaying is shut down.

Is it possible these people are sending anything?

I have been monitoring outbound mail, and have not seen any yet.

How do you prevent them from creating a session on the SMTP server?

If you see nothing in your queues, they seem to be attempts only.

This is quite normal.
