Do anyone setup Outlook Anywhere without ISA and publish CAS to Internet? Is it practical and safe? I have Netscreen firewall and Squid proxy.

I dont see any reason why you cannot publish OL Anywhere without ISA as all traffic is over SSL and encrypted.
