It is possible to set up a white list for IMF to use with Exchange 2003 sp1? Or anywhere in Exchange 2003, for that matter? I can see where you block Senders, but is there an allowed senders list?

Server-side you can only add IP address from which Exchange should accept mail (Connection filtering tab). But if you use Outlook 2003 you can use the Safe Senders list locally in each Outlook client.

Does the message even get to the safe senders(Outlook) list if it is blocked by the IMF?

Not if the message is blocked at the Gateway level, but if it's let through to the Mailbox store then yes the Outlook 2003 Safe Senders list take effect.
