After restarting the exchange 2003 server some users reported receiving old smtp mail that they had already previously received.

Has anyone else experienced this?

I recommend the following:
Backup the server before doing anything...
1) Disable all 3rd party services (Antivirus, backup, disclaimer, antispam, etc) that is on that server.
2) Stop the Info Store service and defrag your DB (192185 XADM: How to Defragment with the Eseutil Utility (Eseutil.exe) - http://support.microsoft.com/?id=192185)
If needed, you can run ISINTEG -FIX however it should not be needed unless MS says otherwise.

3) Apply the latest service pack for your OS and any hotfixes for Exchange 2003.
4) Restart the server (leaving the 3rd party services as disabled)

If that does not clear out the messages, you may want to call Microsoft support to empty your SMTP Mailbox temp tables...
