Meeting request sent to unspecified user

I am at my wits end and hopefully someone can help me.  Let's say User A creates a meeting request to User B and User C.  There is also a mystery User D that gets this request.  They are not listed as a delegate on any of the other Users.  I am not sure how this User D fits in.  Are there any good ways of tracking this down?

This can be caused by corrupt rules. You can verify if this user has a hidden delegate rule that is not visible in Outlook. The steps in this article will go over how to determine this.
