I have one client working on SBS 2003 with a hardware firewall without major problems. If I remember correctly, the SBS Connect to the Internet wizard pretty much took care of all the configuration changes you have to do manually on a regular Exchange 2003 box (I think I verified the changes, but I don't remember having to do anything manually - its been a while). Just make sure "Outlook via the Internet" is checked in the wizard. You should be able to bring up a client-side configuration step-by-step tailored to your domain if you access https://servername/remote , log in, and on the right-hand side, click on Configure Outlook via the Internet. Also, I would make sure you have a third-party SSL certificate issued as opposed to the built-in SBS certificate so you don't have issues with manually installing that certificate on the client side. You can buy a cheap turbo SSL from GoDaddy.
