If it's okay internally, then maybe there is redirect on the ISA server.  I don't expect that internal requests will be going through that.

From what i read here i think the issue is an ISA issue.

What is the internal IP address for www.hospital-vilaverde.com and webmail.hospital-vilaverde.com?

In ISA you can do port forwards but for HTTP and HTTPS you can also user WEBLISTENERS. With a web listener you can redirect http and https requests based on the url that you request.

With web listeners you can redirect traffic to www.hospital-vilaverde.com incomming on port 80 to your webserver (.9) and you can redirect traffic to webmail.hospital-vilaverde.com incomming on port 80 to your echnage/OWA server (.10).

There is a Publish Exchange server wizard build into ISA.

It seems that this server farm is a real mess... I'm doing some tests. I deactived the rule that should allow the web traffic to be redirected form the .9 server to .10 server in ISA. Strangely, externally I still have access to the pages allocated in the .9 server, even with the default rule in ISA set to deny all access. I'm requesting the external dns table to see what's going on here. I start to believe that ISA is doing nothing, but I have to take a look at the physical servers since I'm working on them by remote access.
