You only need TCP 443 open from the Internet to the CAS. Nothing other. At all. Nothing. None.

Where do you have the CAS? It must not be in the DMZ.

Thanks for your replay,
all roles in exchange server (CAS,HUB,MAIL BOX). the exchange located
like the following:
fortigate (firewall)->fortimail(antispam)->Exchange server 2007(CAS,HUB,MAIL BOX).
so what I have to do now.

Since I have no idea where your skills lie I can only point you at:
http://technet.microsoft.com/en-us/library/bb123889(EXCHG.80).aspx to get you started.
You need to look at your Exchange and make sure that you have a certificate that is appropriately trusted so that there is no pop-up warning when you use OWA or OA. That's key.

I don't think it is a certificate issue, I told you it worked when I open all ports.

Yes, and because I don't have sight of your system I told you about the certs since there's a good chance that your Outlook Anywhere (HTTPS) is potentially falling back to RPC.
Read the docs and walk through each step then come back to us with questions about any odd config you think you might have.
