I use a loadbalancer to offload the certificate, so http traffic to CAS servers is enabled. Clients should only access Exchange through the loadbalancer using Https or Rpc.

But users can now also use http when they know the server url.

How do I open http traffic to the Cas servers for the loadbalancer only?
(which firewall rule to create or edit?)

There are multiple ways to achieve this, also depending on the nature of the setup.

A typical one would be to use a two-armed setup and activate the LoadMaster's "Packet Filter" option. Or simply to limit the routing on the CAS servers.

Just though i'd add that most of our LoadMaster customers who are load balancing Exchange create a Virtual Service on the LM that listens on port 80 and sends a 302 http redirect back to the client, so that the request comes back via https.

KEMP makes this easy; once a customer has created their port 443 VS, the PostMaster can auto-create this redirecting VS with a single tickbox.
