Strange Exchange 2010 / Cluster issue

Hi guys,

We have x4 mailbox servers in our DAG. x3 at our main AD site and x1 at our DR AD site. We also have these servers protected by a Windows failover cluster.

Last Thursday we started seeing the following error for the node that sits in our DR site: "Node 'servername' failed to join the cluster because it could not send and receive failure detection network messages with other cluster nodes. Please run the Validate a Configuration wizard to ensure network settings. Also verify the Windows Firewall 'Failover Clusters' rules. We are seeing '1572' events logged on the affected node.

The cluster networks are also reporting as 'down' for this node, but each node can see/ping each other without any issues. We do not have local firewall configured on any of the mailbox servers in the DAG/cluster. Cluster validation report (network check only) fails with some warnings:

Nodes are not consistently configured with IPv4 and/or IPv6 addresses on network adapters
that are usable by the cluster.
Node node1.domain.lan is configured with IP addresses from protocol IPv4 and IPv6.
Node node2.domain.lan is configured with IP addresses from protocol IPv4 and IPv6.
Node node3.domain.lan is configured with IP addresses from protocol IPv4 only.
Node node4.domain.lan is configured with IP addresses from protocol IPv4 and IPv6.

Now, we did have a similar issue with this node a couple of months ago and the solution was to simply re-IP the main NIC on the server - after doing this everything came up ok and the node reported online/replication to the mailbox copies was all OK etc. However, we've already tried this but it didnt work.

We assume this is related to the Microsoft Failover Cluster Virtual Adapter on 'node3'. See below for an example 'ipconfig /all' on a working node and also on the affected node:

Looks like there is fundamentally an issue with the cluster adapter.

I have also generated a cluster log for this node and can provide both this along with a cluster validation report if someone can assist us with this issue?
