for internal mail see if you are getting event errors at same time you are getting the failed email ndr.If you are getting an event error related to DSAccess, then your Domain Controller may be failing intermittently. You can look up issues related to DSAcess and Exchange 2003 on the Microsoft support site. Strategy here would be to isolate network or server failure to find or talk to the Domain Controller/ Active Directory.

Hello guys,
In my case the problem was the 'from' field.
The user is checking multiple mailboxes on Outlook 2003.
When she received a mail in the xxx mailbox (which is not her primary mailbox), she would click 'forward' and automatically the 'from' field is filled with the name of the xxx mailbox (not her name).
I found out that somehow permissions on Exchange had been changed and even though she had 'Send As' permission on the xxx mailbox she would receive the error message every time she forwards a message.
The fix was to edit the xxx mailbox permissions in 'Active Directory Users and Computers' on the Exchange 2003 server and under 'Permissions' click her name and check the allow 'Send As' and 'Receive As' box
